If You schedule an appointment online with us, we may need to collect additional information such as which service You need. If You are a job applicant applying for a job with us, we will collect Your name, email address and information related to Your work history. We may also need to collect Your phone number and home address as well. In the event that You are offered the position, we will need to collect additional Personal Information and Sensitive Personal Information from You in order to offer You the job and onboard You in the same.

XI. How do we Secure and Protect Your Personal Information
